Common questions about the nursing assessment

What does a nursing assessment at home include?
A full visit at home in which the nurse evaluates general condition, skin, mobility, vital signs, medication and the surroundings (bed, bathroom, fall risks), and ends with a written report and a care plan with concrete recommendations. You come out of the visit knowing what’s going on and what to do.
How long does the assessment visit last?
Between 60 and 90 minutes, depending on the complexity of the case: the time needed to examine calmly, talk with the patient and with whoever cares for them, and explain the conclusions without rushing. It is not an express visit.
Do I get a written report?
Yes, always: a clear nursing report with what was found, the recommended care plan and the signs worth watching. It’s useful for the family, for the GP and for applying for support or making decisions.
When is it worth requesting an assessment?
When you notice a change and can’t interpret it: eating less, falling more often, a wound that isn’t improving, medication piling up, or simply “she doesn’t seem herself”. Also after a hospital discharge or before organising care at home: the assessment tells you what’s genuinely needed.
Does the assessment commit me to booking other services?
No: it is a complete service in itself and the report is yours. If the plan recommends wound care, injections or care support, you can arrange it with whoever you like — though of course we’d be delighted to carry on ourselves.
How much does a nursing assessment at home cost?
It has a fixed price we confirm on WhatsApp according to your area, with travel and the report included — noticeably more accessible than an equivalent private medical assessment. The initial consultation is free.
